The Tdap booster will be administered by nurses from Healthy Schools at clinics set up at middle schools and K-8 centers between May 21 and May 31, 2018.

Parents should contact their child’s school for specific dates and times of the clinics. A consent form will be sent home with each student. Parents or guardians may either complete the paper form and return it to the school any day before the clinic day scheduled for their child’s school or sign up at  www.miamiitdap.com/6 between April 30 and May 11, 2018. Children will not be vaccinated without the completed consent form.

The paper consent form will available in English and Spanish. Parents/guardians who would like an additional copy of the paper consent form or require additional languages, please email info@healthyschoolsllc.com.

For more information about Healthy Schools LLC, questions or for assistance in completing the consent form, please call School Operations/ Comprehensive Health Services at 305 805-4600.
